Aetna (PPO), UMR, FFS, HPN Medicaid, Silver Summit, HPN PPO, Sierra Choice, Cigna, SHL, Anthem PPO, Anthem HMO, Anthem Medicaid, Molina, HealthScope, HealthScope PEBP, Electrical Workers, United, Prominence.

Don’t see your insurance provider listed? No problem!

We accept all Preferred Provider Organization (PPO) commercial insurance plans that have Mental Health/Substance Abuse benefits. We can also provide receipts/superbills that you can submit to your insurance company. When doing so, patients typically receive reimbursement for 55-90% of the cost, depending on the insurance provider.

Sliding fee discounts

Sliding fee discounts are available based on family size and income. At Healthy Minds, we don’t believe anyone should be denied services based on their ability to pay.

Your eligibility is based on how your total annual income fits into the federal poverty levels. Please contact us or ask an office administrator for an application.
